Missing Person form
in South Africa, there is NO WAITING PERIOD to report a missing person!
Go to your nearest Police Station immediately to fill out and sign an official SAPS 55(A) form

Bring the following to the station to aid the investigation:
- The missing person’s full name, age, ID number
- A recent photograph and physical description (height, hair color, eye color)
- Any identifying features such as scars or tattoos
- Details of the clothes they were last wearing
- The date, time, and location where they were last seen
Once the SAPS 55(A) form is filed, the investigating officer will provide you with a case number. It is important to obtain the investigating detective’s contact details and keep them updated on any new developments.
NB: If the missing person returns or is found, you must notify the investigating officer immediately to complete a SAPS 92 form and remove them from circulation
